Screening and Eligibility

Start by completing the student intake – then we’ll schedule your appointment

Student Intake

There are 3 parts to the student intake process: A Copy of the Birth Certificate, Verification of Residency and the Parent Intake Packet.  These three pieces MUST be completed before your child can be seen by our staff.

  • You must provide either:  

    • Copy of your child's birth certificate or
    • Legal paperwork (guardianship/temporary custody)

    *Please be advised, the person bringing the child to the appointment must be the legal guardian

  • Option One

    In order for your child to be evaluated, a parent/guardian must show proof of Orange County Residence.  This can be in the form of on of the following documents:

    • Current homestead exemption
    • Property deed
    • Property taxes
    • Title
    • Current Lease

    *If you do not have a lease in your name or own your own home and live with someone else, please continue to option two below.*

    Option Two

    If you do not have a lease in your name or own a home and you live with someone else, you must schedule an appointment for Address Verfication with OCPS Student Enrollment- 6501 Magic Way, Bldg 100B, Orlando, FL 32809

    For Option Two, Parents must have: 

    1. Parent or guardian’s Florida driver’s license or state ID showing the current correct address.

    2. Another item in the parent or guardians name showing the current address such as: auto registration, current bill or current pay stub

    3. Homeowner or lessee’s homestead exemption card, property tax information or lease agreement

    4. Homeowner or lessee’s Florida drivers license or state ID showing the correct address.

    *Schools will only accept an address verification form that has been notorized by a staff member in the Student Enrollment Office.*

     

     

  • There are five forms included in the Parent Intake Packet

    1. Parent Intake Form: Provides us developmental and medical history about your child
    2. Preschool/Child Care Input Form: Provides us information on how your child participates at child care/ preschool
    3. Home Language Survey: Provides us information about other languages spoken in the home
    4. HIPAA Form: Allows us permission to contact you regarding appointments, reminders and share information with providers (your child’s doctor, therapist, daycare, etc.)
    5. Release of Information Form: Gives us permission to request information from providers (your child’s doctors, therapists, child care, etc.)

    Additional Information

    • An incomplete packet may delay your appointment date
    • We ask parents to  include any medical reports or testing that would help with the evaluation process. 
    • If you child is hearing and/or visually impaired please send in the most recent audiogram (aided and unaided) or ABR and/or their vision report.
